Healthcare provider forms and guides

The Healthcare Provider Portal or integrating your clinical software provides you alternate online channels to interact with the NCSR to eliminate paper, fax and phone calls, while also making it easier for healthcare professionals to submit information.

For example, if you are submitting information such as a Bowel GP Assessment form, you will not need to fill out patient details as they will be pre-populated.

The importance of reporting

Healthcare providers play a key role in delivering clinically appropriate advice, services, treatment and care while ensuring that program participants progress through the screening pathway.

By completing and submitting information forms, you are providing essential data on participants and their outcomes to the NCSR. 

Updated Participant Details and Alternative Access to Kits Model forms

Changes have been made to simplify these forms.

 

When issuing a kit to a patient through the Alternative Access to Kits Model, you will now see a shorter form with a new name.

 

The previous form called NBCSP – Alternative Access Model (Issue/Re-Issue Kit) is now called NBCSP – Bowel Kit Issued by Healthcare Provider

Proceduralists use this form to provide information about your patient’s colonoscopy and histopathology result.

General practitioners (GPs) use this form to provide information about consultations with bowel cancer screening patients who have received a positive iFOBT test result.

Histopathology Report form

Histopathologists use this form to report patient specimen results collected during a surgical resection for a patient of the program. To report results from all other procedures, please use the colonoscopy and histopathology report.

Procedure Report – Adverse Events form

Colonoscopists use this form to report adverse outcomes of diagnostic investigation procedures for NBCSP patients who have received a positive iFOBT result. Only use this form if the information has not already been included in the colonoscopy and histopathology.

HCP cervical screening history request fax template

If you have not gained access to the Healthcare Provider Portal, Healthcare providers can use our fax template to request cervical screening histories for the purpose of managing your patients’ cervical screening. Healthcare providers can request screening histories for up to 20 patients per day.

Colposcopy & Treatment Form

Colposcopists and other healthcare providers who perform colposcopies are required to notify prescribed cervical screening information to the Commonwealth Chief Medical Officer (CMO) through the NCSR.

Pathology Laboratory Fax Template

If you have not gained access to the Healthcare Provider Portal, use our fax template to request cervical screening history requests for the purpose of cervical screening management.
